To make an appointment with Dr. Yuska at his clinic near East Towne Mall, please call
the clinic at (608) 663-4550.
Appointment times are also available at the Columbus Clinic each week. Please call the
Columbus Community Hospital’s reservation desk at (920) 623-2200 for available
appointment dates and times.
How to Get to the Office
Dr. Yuska’s Office
Dr. Yuska’s office is located in the M&I Bank building at 4726 East Towne
Boulevard on the corner of East Washington and East Springs Drive.
If you are on I-90, exit at 135A and turn left at the first light (East Springs Drive).
Take the first right on East Towne Boulevard. The M&I Bank building parking lot is on
the right. Dr. Yuska’s office is on the first floor.
Meriter Hospital

Columbus Community Hospital
Columbus Community Hospital is located 10 minutes past Sun Prairie,
Wisconsin, about 15 minutes from the east side of Madison. On Highway 151 take the first
Columbus exit and turn right. The hospital is on your right-hand side. Use the main
entrance for the second floor clinic.
